Just some important reminders (all of which are also on the form):

- Do not mail the form to me, mail it to

- Include your address on the form to receive your tax receipt

- If you are paying by check, please include rider name (Jonathan Fish) and rider number (5771) on your check

- If you are paying by credit card, you must include your phone number

- If you are having your pledge automatically deducted from your checking account, you must include a voided check when you send in your pledge form.

- Pledges are immediately utilized by the beneficiary
